- Cash payments – fill out LETTER OF INTENT and return to your Coach or Coach Cleveland
- Check payments – fill out LETTER OF INTENT and turn in three checks (dated 9/23, 10/23 and 11/23) and return to your Coach or Coach Cleveland
- Credit Card payments – fill out LETTER OF INTENT and CREDIT CARD FORM and return to your Coach or Coach Cleveland
